Getting Started with Forms

Clevada Forms lets you build custom forms and surveys, collect responses, and analyze the results - all without any coding.

Creating a Form

  1. Go to Workspace and click the Forms icon in the sidebar
  2. Click + New Form
  3. Give your form a title and click Create

Building your form

The form builder lets you add fields by clicking + Add Field. Available field types include:

  • Text fields - short text, long text, email, phone, URL
  • Choice fields - multiple choice, checkboxes, dropdown
  • Scale fields - rating (stars), NPS (0-10), linear scale
  • Date and time - date picker, time picker
  • File upload - let respondents attach files
  • Content blocks - headings, paragraphs, dividers for structure

Drag fields to reorder them. Click any field to edit its label, description, and validation rules.

Multi-page forms

Enable Multi-page mode in form settings to split your form into sections. Each section becomes a separate page with a Next / Back navigation.

Sharing your form

Once your form is ready, click Share to get:

  • A public link - forms.clevada.com/f/your-form-slug
  • An embed code - paste into any website as an iframe
  • A private token link - for internal use without public indexing

Viewing responses

Click Submissions on your form to see all responses. You can:

  • Filter by status (New, Read, Starred, Spam, Archived)
  • Bulk-change status on multiple submissions
  • Export all responses to CSV

Notifications

In Settings > Notifications, enter one or more email addresses to receive an email every time someone submits your form.

Closing a form

To stop accepting responses, go to Settings and either:

  • Toggle off Accepting responses
  • Set a response limit or a close date
